It's just based on strength of will until your mind shuts down. You can will yourself to not react, it's not turning off the pain or the suffering, just controlling your emotions when it the worst of it strikes.
Just as Upon An Oaken Throne said, it's tolerance. Some people just have higher thresholds of pain as well.
