You're right on your first 2 points. The song doesn't refer to two people only, and love does't have to be between 2 people. But i think the second line in the song about "Self loathing" refers to the writer. He was empty and hated himself before he met this particular person/group of people.
As far as the overall meaning of the song, I think Matt was trying to make a heavy love ballad (as i stated earlier). I could be wrong, but that's a risk i'm willing to take.

The album "The Crusade" tackles social and eithical issues so it would make sense if the song were about things on a far grander scale.
